On this page you will find some useful paperwork. Please print out the first three and fill them out
completely and bring them with you to our first meeting.

This form should be completed if you plan to use a credit/debit card or electronic check.

These release forms should be completed if you would like me to be able to get information from
your doctors, or if you'd like me to send information to your doctor.
